แบบขอความยินยอมสำหรับการประมวลผลข้อมูลส่วนบุคคล

เรียน ผู้เข้าร่วมและผู้ประสงค์จะเข้าร่วมการสัมมนาหรือโปรแกรมการฝึกอบรม

บริษัท นิโปร เซลส์ (ประเทศไทย) จำกัด (“บริษัทฯ” หรือ “เรา”) ได้จัดตั้ง สถาบันเพื่อการประกอบวิชาชีพทางการแพทย์ (“iMEP”) เพื่อให้บริการโปรแกรมเกี่ยวกับการศึกษาและการพัฒนาวิชาชีพ ที่ออกแบบมาเพื่อส่งเสริมความรู้ ทักษะ และความเชี่ยวชาญของพยาบาล วิศวกรคลินิก แพทย์ และบุคลากรทางการแพทย์อื่น ๆ รวมถึงบุคคลดังกล่าวที่ประกอบวิชาชีพอยู่ในประเทศไทย

เพื่อให้บริษัทฯ ปฏิบัติตามพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562 (ค.ศ. 2019) และกฎหมายลำดับรองของพระราชบัญญัติดังกล่าว (ซึ่งต่อไปนี้เรียกรวมกันว่า “PDPA”) บริษัทฯ ใคร่ขอความยินยอมจากท่านในการเก็บรวบรวม ใช้ และประมวลผลข้อมูลส่วนบุคคลของท่าน สำหรับวัตถุประสงค์ดังต่อไปนี้

  • เพื่อเชิญท่านเข้าร่วมการสัมมนาหรือโปรแกรมการฝึกอบรม การประชุมเชิงปฏิบัติการ กิจกรรมด้านการศึกษา หรือกิจกรรมอื่น ๆ เพื่อพัฒนาความรู้และทักษะทางวิชาชีพในอนาคต ซึ่งจะจัดขึ้นหรือได้รับการสนับสนุนโดยบริษัทฯ ทั้งนี้ เท่าที่กฎหมายที่ใช้บังคับอนุญาต
  • เพื่อแจ้งข้อมูลข่าวสารและข้อมูลสำคัญเกี่ยวกับการสัมมนาหรือโปรแกรมการฝึกอบรม กิจกรรมด้านการศึกษา หรือกิจกรรมอื่น ๆ ที่บริษัทฯ จัดขึ้น

การให้ความยินยอมของท่านขึ้นอยู่กับความสมัครใจของท่านเท่านั้น ท่านมีสิทธิตาม PDPA ในการปฏิเสธการให้ความยินยอม หรือถอนความยินยอมได้ทุกเมื่อ โดยการบล็อกหรือลบบัญชี LINE Official Account ของบริษัทฯ หรือโดยติดต่อบริษัทฯ ตามรายละเอียดการติดต่อที่ระบุไว้ด้านล่าง

โปรดทราบว่า การตัดสินใจของท่านในการไม่ให้ความยินยอม หรือการถอนความยินยอม จะไม่ส่งผลกระทบต่อสิทธิของท่านในการลงทะเบียนหรือเข้าร่วมการสัมมนา โปรแกรมการฝึกอบรม และกิจกรรมใด ๆ ที่ท่านได้ลงทะเบียนไว้แล้ว หรือมีสิทธิเข้าร่วมอยู่แล้ว อย่างไรก็ตาม หากท่านไม่ให้ความยินยอม หรือถอนความยินยอมในภายหลัง บริษัทฯ อาจไม่สามารถส่งคำเชิญ ประกาศ ข่าวสารและข้อมูลอัปเดต หรือข้อมูลอื่น ๆ เกี่ยวกับการสัมมนา โปรแกรมการฝึกอบรม กิจกรรมด้านการศึกษา หรือกิจกรรมอื่น ๆ ที่บริษัทฯ จัดขึ้นในอนาคตที่อาจเป็นประโยชน์หรืออยู่ในความสนใจของท่านได้

Dear Participants and Prospective Participants of Training Programs,

Nipro Sales (Thailand) Co., Ltd. (“Company” or “we”) has established the Institute for Medical Practice (“iMEP”) to provide educational and professional development programs designed to enhance the knowledge, skills, and expertise of nurses, clinical engineers, physicians, and other healthcare professionals, including those practicing in Thailand.

Pursuant to the Personal Data Protection Act B.E. 2562 (2019) and its subordinate regulations (collectively, the “PDPA”), the Company hereby requests your consent to collect, use, and otherwise process your personal data for the following purposes:

  • To invite you to participate in future training programs, seminars, workshops, educational events, or other professional development activities organized or supported by the Company, where permitted under applicable law.
  • To provide announcements and important notifications relating to training programs, educational activities, or other events organized by the Company.

Your consent is entirely voluntary. You have the right under the PDPA to refuse to provide your consent or to withdraw your consent at any time by blocking or deleting our LINE Official Account or by contacting the Company using the contact details provided below.

Please note that your decision not to provide, or to withdraw, your consent will not affect your eligibility to register for or participate in any training program or event for which you have already registered or are otherwise entitled to attend. However, if you do not provide your consent, or if you subsequently withdraw your consent, the Company may be unable to send you invitations, announcements, updates, or other information regarding future training programs, educational activities, and events that may be of interest to you.
